Kyle Firstenberg mentions Jon Phelps as having role on the Board of Advisors and Accountability at Mars Hill, publishes charges re: Driscoll and Turner

... The second major change confirmed from Mark’s own mouth that he holds the power to make changes, not only to the Bylaws, but across the board with no accountability. This happened in 2011-2012 when there was another proposal to change the Bylaws to incorporate the Board of Accountability and Advisers, consisting of Jon Phelps, Michael Van Skaik, James MacDonald, Larry Osborne, Mark Driscoll, Dave Bruskas, and Sutton Turner

This is worth noting because it's the first time someone seems to have stated that Jon Phelps has had any role on the BOAA who can be identified by name.  There are anonymous comments here and there and informal proposals that Jon Phelps has had a role on the BOAA but up until recently there were no statements to that effect from people with names and there's been no public statements from Mars Hill Church in the last few months even addressing whether or not Jon Phelps has had any role on the Board of Advisor's and Accountability.

Firstenberg has republished what look to have been formal charges he made regarding Mark Driscoll and Sutton Turner as pastors at Mars Hill Church over here:

POSTSCRIPT 03/29/2013

For those who want more context for Jon Phelps' historic connection to Mars Hill Church, largely recounted here and there by Mark Driscoll himself and with a little election-year coverage from Rachel Madow ... over here:

It was mentioned by an anonymous commenter that Phelps was on the board and now it appears someone with a name has publicly linked Phelps to the accountability board that now seems to be known as the Board of Advisors and Accountability.  If Mars Hill Church and the BOAA itself can clarify this they're certainly welcome to address what Phelps' role in the history of MH has been and may continue to be.
